The Hues Hope Medical Clinic & Wellness Center is more than a clinic. It is a healing environment embedded within Tower One — designed specifically for residents living with complex medical, mental health, and substance use challenges who have historically been underserved by traditional healthcare systems.

Who Will Be Able to Access the Clinic
- All Tower One residents — always, at no cost
- Tower Two residents via referral from case manager
- Community members on a sliding-scale fee basis
- Walk-ins will be welcome for urgent care needs
- Anyone referred by Hennepin County or partner agencies

Primary Care
Comprehensive primary care for chronic and acute conditions, delivered by a dedicated care team who knows you by name.

Behavioral Health

Initial Health Assessment
Every new resident receives a comprehensive health assessment within 72 hours of move-in to identify needs, medications, and care priorities.

Personalized Care Plan
A dedicated care team develops a tailored health plan in collaboration with the resident, incorporating their preferences and goals.

Benefits Enrollment
We assist every resident in enrolling in Medicaid, Medicare, and other benefits they are eligible for — often unlocking resources they didn’t know existed.

Transportation to Appointments
Our outreach team coordinates transportation to specialist appointments, hospital visits, and off-site care to eliminate a major barrier to follow-through.

Ongoing Case Management
Each resident is assigned a dedicated case manager who monitors housing stability, health progress, and social needs with regular check-ins.

Our planned clinic will operate entirely within the Housing First framework — a nationally proven model that prioritizes immediate access to permanent housing without preconditions, coupled with voluntary supportive services.
What Housing First means in practice:

- ✓ All services are voluntary and resident-directed
- ✓ Staff are trained in motivational interviewing & de-escalation
